CURRENT FORM COVERAGE
Know what’s available.
The included in-app form is California MC-030 (Declaration). Other state forms, including MC-031, FL-300, FL-320, and FL-150, link to official sources. Compatible fillable PDFs can be uploaded; scanned or unsupported PDFs cannot be filled in the editor.
The county selector covers California. Dedicated local source links currently cover Riverside, Los Angeles, Orange, San Diego, and San Bernardino counties. Other selections point to the official court directory. This is not a complete form collection for every county or state.
Official court forms are free. Your membership pays for the editing, saving, and preparation tools.

Do I need to pay for the free kit or timeline?
No. The downloadable kit and The Record timeline are free. The $20/month membership is for Forms Desk document and PDF editing.
Is a lawyer or filing service included?
No. Forms Desk is organizational software. Legal advice, court filing, representation, and therapy are not included. It does not determine which form you need or guarantee court acceptance or an outcome.
What happens when I cancel?
You can cancel future renewals through Manage membership. Paid editing continues until the end of the paid period. Your existing drafts remain available to view and export. See the cancellation policy for refund requests and details.
How does checkout work?
Sign in to Forms Desk, review the $20 monthly renewal and policy terms, and select both consent checkboxes yourself. Continue to Stripe to complete payment. Opening the review does not charge you.
